What is DHI?

DHI, Direct Hair Implantation, is a refinement of FUE in which extracted grafts are loaded into a Choi implanter pen and placed directly into the scalp without pre-opened channels. The pen creates the opening and deposits the graft in one controlled motion, which gives the surgeon direct command over three things that decide how natural a transplant looks: the angle each hair exits the skin, the direction it points, and the depth it sits at.

That control matters most where the eye is least forgiving, the hairline, the temples, partings and any zone where transplanted hair meets existing hair. Because the pen can work between existing hairs with minimal trauma, DHI is also the technique that makes unshaven or partially shaven procedures possible for suitable cases.

How it works at BlueMagic Group Clinic

Extraction is identical to FUE: follicular units are removed one by one from your donor area under local anaesthesia. The difference is what happens next, instead of opening channels first and implanting second, trained implanters place each graft directly with the Choi pen while the surgeon controls design and distribution. A 6–8 person medical team runs every procedure.

DHI is recommended after a medical review of your photos. If your case is better served by Sapphire FUE or a combination, that is what we will advise.

Recovery and growth timeline

  • Days 1–3: mild swelling and redness; sleep semi-upright on your back.
  • Day 2–3: first wash at the clinic, done with you, then continued daily at home for 10 days.
  • Days 7–10: crusts lift away; the implanted area looks clean.
  • Weeks 2–8: shock loss, transplanted hairs shed. Normal and expected.
  • Month 3: incubation ends and new growth begins.
  • Month 6: visible result of at least half the expected growth.
  • Months 12–18: final density and texture, when results are formally assessed.

Risks and possible complications

DHI under local anaesthesia is a low-risk procedure, but no medical procedure is without risk. Possible effects include temporary swelling, numbness, itching, folliculitis and, rarely, infection or reduced graft survival. Smoking, alcohol and certain medications impair healing; you receive clear pre-operative instructions to control every controllable factor, and an emergency line to call if anything concerns you after the procedure.

We would rather decline a case than transplant a patient whose donor area or health profile makes a good result unlikely.

DHI, frequently asked questions

Why does DHI cost more than Sapphire FUE?

DHI is more operator-intensive: grafts are loaded and placed individually with implanter pens, which takes more specialist hands per graft. You are paying for placement control, which matters most in hairline and detail work.

Is DHI really scarless?

No technique is scar-free, and we will not claim otherwise. DHI leaves the same micro-dot donor marks as FUE, tiny, scattered and hidden by surrounding hair at normal lengths. There is no linear scar.

Can DHI be done without shaving my head?

In select cases, yes, typically smaller sessions where the donor area can be shaved in concealable strips and the recipient area is worked between existing hair. Whether your case qualifies is decided in the photo assessment.

How many grafts can DHI place in one session?

Sessions are moderate compared with FUE because each graft is placed individually. If your plan needs very high numbers, the team may recommend Sapphire FUE, a combined approach, or staging over two sessions.

Does DHI grow faster than FUE?

No. Growth biology is identical: shedding in the first weeks, new growth from month 3, at least half the expected result by month 6, and final assessment at 12–18 months. The difference between techniques is placement control, not speed.

Is the result permanent?

Transplanted follicles come from the donor zone that is genetically resistant to pattern loss, so they are long-lasting. Native hair around them can keep thinning, which is why the plan accounts for future loss and may include medication advice.
